Upload your report through the “Forms” section in Extended Reach.

Submit within 24 hours of any:

• injuries
• behavioral incidents 
• safety concerns
• medication errors
• runaways
• disputes or conflicts
• property damage
   

If it affects the child’s safety or stability, please report it.

 Upload using the “Incident Report” tab in Extended Reach
Call your CPMS immediately in emergencies

Upload certificates for all required DFPS trainings:

• Trauma-Informed Care
• Medical Consent 
• Psychotropic Medication Training 
• Normalcy 
• CPR/First Aid
   

Use the “Training” tab to upload PDFs or screenshots of certificates.

 Document:

• Appointments
• Diagnoses
• Prescriptions
• Medication administration 
• Hospital visits 
• Specialist referrals
   

Enter updates under the “Medical” tab.

Document all:

• phone calls
• school meetings 
• caseworker visits 
• therapy appointments 
• interactions with biological family 
• significant conversations or updates
   

This supports accurate case documentation.

Extended Reach stores each child’s:

• Placement paperwork
• Service plans 
• Medical consents 
• Educational records
• Court documents
• Visitation schedules
   

You will have limited access to the documents relevant to the child in your care.

 Do I have to use Extended Reach?

Yes!  it is required for documentation compliance and DFPS monitoring.

What if I forget my password?

Click “Forgot Password” on the login page or email support@OneTribeCPA.com.

Can I use my phone or tablet?

Yes!  Extended Reach is mobile-friendly.

Where do I upload monthly reports?

Under the “Forms” tab inside your portal.
